Transcript Request

Below is the transcript request form for Norfolk Collegiate Alumni to use to request records for schools and employers, etc. Completed forms will be electronically sent to the Norfolk Collegiate Registrar for processing. Transcripts will be mailed via U.S. Mail. to the requested addresses.


Should you prefer to complete a paper version, please print and complete the Transcript Request Form (PDF). Please use one form per recipient or address. The Transcript Request Form(s) must be signed and faxed to 757.299.2591 OR mailed to the following address:

High School Transcript Request Form
The transcript request form should be completed, signed and submitted to the Office of the Registrar by fax or mail. Registrar address information is listed at the bottom of the form. Transcripts will be mailed via U.S. Mail. NCS MUST have the signature of the former student to release transcripts. A parent or legal guardian may sign only if the former student is under 18. Please use multiple forms if transcripts are to be sent to more than one address. Please allow 5-7 business days for processing from the time the request is received in our office.
